General Terms: I authorize MediGraph to charge the credit card listed below on a monthly basis, for fees that are generated by using the software. Monthly statements will be available in the ‘My Account’ section of the MediGraph website, for up to two years. I further understand that in the event that my account goes into a past due status, access to the service may be restricted. If I should cancel my subscription, I will no longer be able to use the service. It is advised to print / export all records before discontinuing service. All data will be permanently removed from MediGraph’s computers within 30 days of cancellation. Rates and terms are subject to change in the future. Prior written or electronic notice of any change will be provided.
Minimum Monthly Fee: A minimum monthly charge of $75 is applied against your visit, billing, and FCE usage.
Software Upgrades: Software updates are made periodically, and are free of charge to all our subscription customers.
MediGraph Central: MediGraph Central has a cost of $50 per month. This covers the cost of hosting your data, as well as daily backups.
Backup Service (MediGraph Desktop Only): Should you elect to use our secure internet backup service, you will be billed an additional $15 per month.
Backup Service / Service Level Agreement (MediGraph Central Only): MediGraph will make every attempt to have the service available at all times. Should MediGraph be unable to maintain a monthly uptime of 99%, the monthly maintenance fee will be discounted by 20% for each day affected. MediGraph will create daily backup copies of the client’s data, which will be retained for at least 7 days. Should a restoration be needed due to failure of equipment, no charge will apply. Should the client request a restoration due to staff error (deleted records, overwritten content, etc.), a $75 fee will apply.
30 Day Satisfaction Guarantee / Contract Term:
If you are not satisfied with MediGraph within the first 30 days of your subscription, you may cancel your subscription with no charges. Your subscription will automatically continue on a month-to-month basis, unless MediGraph receives notification of cancellation.
Training / Software Upgrades / Technical Support:
MediGraph is pleased to offer training, software upgrades, and technical support to our customers. Training regarding the use of the software is available free of charge. Software updates are made periodically, and are free of charge to all our customers. Technical support directly related to MediGraph is provided free of charge. Support for peripheral issues, such as networking, clearinghouse integration, and customer-owned equipment is subject to a fee.
